A step-down forensic supported Living service with a focus on recovery, rehabilitation and relapse prevention; developed within an outcomes based risk management framework. the service operates within the category of learning disabilities and mental health and has a complex needs specialism in supporting the progress of individuals whose mental health is affected by personality disorder, anxiety disorder, schizotypal disorders, depression, and substance misuse backgrounds. We have excellent working relationships with a range of external agencies including, Probation, MAPPA, MDT and assertive outreach teams.

At Expanding Horizons our Individual Support Plans aim to provide the people we support with greater participation and more choice and control over their lives. Encouraging people to have a belief in reaching their full potential regardless of their ability within a structured, holistic environment. Our staff have a wealth of experience and expertise in supporting people with forensic needs; including people subject to section 17 leave arrangements.


A 4 bed semi-detached classic Victorian property, situated on the outskirts of the World Heritage town of Blaenavon, Cwmavon Rd aims to provide a warm, homely and therapeutic environment where people can build trusting, dependable relationships within a community setting that focusses on understanding and supporting their individual needs. The property consists of a large kitchen where service users are supported to develop life skills in areas such as food preparation, cooking and laundry. There is a large living room and a separate dining / quiet room area for use at times when service users require their own space, or an area that can be utilized as part of the de-escalation procedure at times of conflict.

Each service user (Maximum 3) has there own furnished bedroom and access to all communal areas of the property. All rooms are tastefully decorated and furnished to a high standard; service users are actively encouraged and supported to take pride in themselves and their home.

With a range of local shops, fantastic outdoor opportunities such as hiking, mountain biking, and visiting World Heritage sites such as Big Pit and The Iron Works; local leisure centre and gym  facilities and excellent transport links to Abergavenny, Cwmbran, Newport and Cardiff, Blaenavon offers a real opportunity for people to be part of community life.

Where appropriate, service users are supported to access education, training and employment related opportunities as part of the aim to support people to reach their full potential.

The staff team at Cwmavon Rd have a wealth of skills, knowledge and expertise in supporting people with mental health issues, learning disabilities and a range of complex health and social care needs. The range of specialist training undertaken by staff includes:







Where appropriate, and dependent upon service users individual needs additional specialist training will be provided to ensure that the skills and knowledge of staff matches the needs of the people we support.


Our team is vastly experienced in supporting people with highly challenging and complex needs to engage in positive behaviours. We use a Positive Behavioural Support approach to developing individual support and risk management action plans. We regularly review each person’s Positive Behaviour Support plans in response to any behavioural change. Each person is supported to develop an Outcomes Based Care Plan designed to meet their individual needs. Our Behaviour Management Specialist works alongside staff teams and multidisciplinary care teams to develop appropriate reactive strategies to maintain placements when challenges arise.


This service model encourages service user’s involvement in every aspect of their life. Active support is an outcomes focussed enabling approach that has led to many people making real choices and positive life changes in their journey towards reaching their full potential.
