Our process
A simple start to your journey

Submit a Referral
Complete our online referral form or contact us, and we will ask you for some details.
Review & Acknowledgement
Our team reviews each referral and confirms receipt within one working day, requesting any extra details if needed.
Initial Needs Assessment & Recomendations
We will visit you to carry out a thorough initial needs assessment. Following this, we will make recommendations about your support needs and possible placement options. (We have a slightly different process for urgent referrals — please contact us and we can help).
Placement Confirmation & Transition
When appropriate placements, support levels, and funding have been agreed, we will work with you to develop a transition plan that works for you, ensuring a smooth and supported start to your journey with us.
