A Supported Living service within the category of learning disabilities and mental health with a complex needs specialism in supporting the progress of individuals with Autism, Asperger’s Syndrome and other associated conditions. Our focus on support, recovery and rehabilitation is developed within an Outcomes Based Framework integral throughout the assessment and care planning process.

At Expanding Horizons our Individual Support Plans aim to provide the people we support with greater participation and more choice and control over their lives. Encouraging people to have a belief in reaching their full potential regardless of their ability within a structured, holistic environment. Our staff have a wealth of experience and expertise in supporting people who have a dual diagnosis of mental health or challenging needs associated with their learning disability.

POSITIVE BEHAVIOUR SUPPORT

Our teams are vastly experienced in supporting people with highly challenging and complex needs to engage in positive behaviours. We use a Positive Behavioural Management approach to developing individual support plans and risk assessments. We regularly review the person’s Positive Behaviour Support plans in response to any behavioural change. Each person is supported to develop and Outcomes Based Care Plan designed to meet their individual needs. Our Behaviour Management Specialist works alongside staff teams and multidisciplinary care teams to develop appropriate reactive strategies to maintain placements when challenges arise.

This service model encourages service user’s involvement in every aspect of their life. Active support is an outcomes focussed enabling approach that has led to many people making real choices and positive life changes in their journey towards reaching their full potential.
